What is a Visit Summary? A Visit Summary is the clinical documentation that results from each visit with your provider. By having access to this information, you can make informed decisions about your health and be an active participant in the management of your care. 

What is contained in a Visit Summary? 

  • Your problem list (what your provider has diagnosed as your medical issues)

  • Results of labs and imaging studies
